Answer Some Questions

Before you get help from us, you need to get an MMO ID card. Before you get your ID card, you will need to prove that you are eligible for our programs. So the first time you visit us, we will have three questions for you:

  • What is your household income?
  • Where do you live?
  • How many people are in your family?

On your first visit, you MUST bring indentification and documents that prove your income and residence. This is what we accept for proof of income and residency:

Identification:

  • Driver's license OR copy of birth certificate AND
  • Social Security Card

Income

  • A recent pay stub OR
  • Documentation from DHS OR
  • A copy of your filed income tax form.

Residence

  • A recent bill with your current address OR
  • A rental lease agreement with your current address.

Your income eligibility will depend on how many people are in your family. Please note: to qualify for Food Pantry assistance, you must live in Washington County, Osage County or Nowata County.
